Summary:In the current article, new clinically relevant aspects of the updated German, Austrian, and Swiss Onkopedia guideline as well as the S3 guideline for the treatment of locally advanced rectal cancer are summarized. Important aspects include (i) the use of total neoadjuvant therapies (TNT) in rectal cancer with high-risk characteristics, (ii) treatment with neoadjuvant chemotherapy in patients with low risk for local recurrence, (iii) immunotherapy with dostarlimab in patients with high-grade microsatellite instability/deficient mismatch repair (MSI-high/dMMR) rectal cancer, and (iv) use of organ-preservation treatment concepts. The availability of several evidence-based treatment options requires intensive discussions within the multidisciplinary team and comprehensive patient information about treatment goals, options, and risks of the various treatment approaches.
