Summary:All patients with medullary thyroid carcinoma (MTC) should undergo calcitonin (CT) determination at regular intervals after total thyroidectomy. Within 6 weeks after operation serum CT measurement has to be performed to determine whether residual tumor or metastases are present (Fig. 1). Patients with normal basal and pentagastrin-stimulated CT levels suggest a tumor-free state where no further treatment is necessary. They can then be followed-up at half-year intervals with physical examinations, and once a year a pentagastrin stimulation test has to be done (Grauer et al. 1990).
