Summary:Purpose: To compare visual and refractive outcomes and monocular defocus performance between the Isopure 1.2.3 (ISP; PhysIOL, Liège, Belgium) and the TECNIS Eyhance ICB00 (EH; J&J Vision, Santa Ana, CA, USA) intraocular lenses (IOLs). Methods: Eyes undergoing uncomplicated cataract surgery with either ISP or EH were reviewed. Preoperative characteristics, postoperative uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), intermediate VA at 66 cm, near VA at 40 cm, refractive outcomes, and monocular defocus curves were evaluated 3 months after surgery. Results: A total of 69 eyes from 51 patients were included (ISP: 31 eyes from 22 patients; EH: 38 eyes from 29 patients). The mean age was comparable between groups (ISP: 64.8 ± 9.0 years; EH: 64.7 ± 8.1 years). Preoperative parameters showed no relevant differences between the two groups. Distance, intermediate, and near VAs, as well as refractive predictability indices did not differ significantly between groups. Both IOLs showed an enhanced monofocal defocus profile, but EH demonstrated a flatter VA decline in the near-defocus range, resulting in better functional near performance. Conclusion: ISP and EH provided comparable distance, intermediate, and near VA and similar refractive predictability. EH demonstrated better functional near vision in the near-defocus range.
