Summary:The landscape of drug-delivery methods in oncology is evolving, with increased availability of subcutaneous (SC) drug administration options. SC injections reduce treatment duration and enhance patient comfort and satisfaction. However, opportunities exist to improve administration modalities, including enhancing practice efficiency and patient experience, addressing needle-phobia, and reducing physical strain for HCPs related to manual injection. Isatuximab (Isa), an anti-CD38 monoclonal antibody approved for intravenous administration in multiple myeloma (MM), is being investigated across the MM treatment continuum for SC delivery via an innovative on-body injector (OBI; Isa SC OBI). With a small, retractable needle enabling hands-free delivery, the Isa SC OBI has the potential to address gaps in the MM treatment experience. We conducted a literature search of MEDLINE/PubMed through 2025 for data on the use of an OBI in oncology (through Isa SC OBI), examining its advantages over SC manual or intravenous administration, considerations for clinical practice, ongoing and upcoming clinical trials, and future MM treatment opportunities. The use of an OBI for the delivery of SC isatuximab could potentially improve clinical workflow, reduce duration of clinical visits and physical burden for HCPs through hands-free administration, and enhance patient experience through the small, hidden needle. Treatment for multiple myeloma (MM) has improved, with more options available for how medications are taken, including delivery of some medicines under the skin (subcutaneous, or SC). Compared with medicine delivered by intravenous (IV) infusion, SC delivery does not take as long to administer-benefiting both patients and their healthcare providers (HCPs)-and can potentially be more comfortable for patients. However, there can be some barriers to SC administration, including fear of the injection needle, side effects at the site where the medicine is administered, and potential cost considerations. Traditional SC delivery also requires an HCP to manually push the medicine slowly through a syringe throughout the administration, which can be physically straining. Isatuximab (Isa), a medicine for MM, is approved for IV delivery. It is currently being studied for SC delivery by an innovative device called an on-body injector (OBI). This delivery method is called Isa SC OBI and is intended to address the above mentioned problems. The Isa SC OBI delivers a set dose of Isa through a hidden needle via a hands-free device; starting the injection requires a simple push of a button by an HCP. Here, we review published data and new studies on the Isa SC OBI, potential benefits and considerations, and what this could mean for the future of MM treatment. Compared with traditional SC delivery methods, Isa SC OBI can help improve both the treatment experience for patients with MM and the clinical experience for their HCPs.
